Medical Form

Each rider must have a current medical form on file at CTRC to ride.

A rider’s medical form must be updated yearly. Riders will not be allowed to ride without a completed form. Medical forms must be thoroughly filled out, signed by a physician and dated to be valid. Forms may be submitted to CTRC via mail, fax (303-652-2072) or handed in at CTRC.

If the rider’s medical form has expired before the start of a session and CTRC does not have a current medical form on file the rider will not be able to participate until a current medical form is submitted. Same applies to any rider whose medical form expires during the course of a session. The rider will be permitted to ride up until the medical form expiration date and not beyond until a new medical form is on file.

In addition if the participating rider has a Seizure Disorder a Seizure form will need to be completed and returned to CTRC along with the medical form.
